Weekly Exercise 18a: Read all of the steps before you begin
  1. Create a form from scratch.
  2. The title and subject matter of this form with My Guest Book.
  3. Save the file as guestbook.htm in your exercise folder
  4. Link to the file from your exercise index page
  5. Contents of the Form:
    1. Use the example on page 345-346 in your book
    2. You will end up with the following input parts on your form
      1. Hidden data
      2. Text
      3. Check Boxes
      4. Radio Buttons
      5. Selection List - Single line and 3 line area
      6. Text Area
      7. Submit Button
      8. Reset Button
    3. Create your own questions!! Be creative.
    4. Include your name as a mailto link at the bottom of the form.
    5. Use this address to "post" your form
      1. http://w5.siskiyous.edu/mailer/mailerSS.asp
    6. Use your own email address in the "sendto"
    7. Use this address in the "from"
      1. postmaster@mailer2.siskiyous.edu
    8. Use your home page address as the "response_url"
  6. Fill out the form and submit. Check your email to find your form submission. When you have made sure everything works, fill out the form and submit, check your email, and then forward the email to the instructor. Include in the subject - Weekly Exercise 18a Completed. If you use the link provided the subject will automatically be entered.

Helpful Thoughts & Ideas:

  • Did you name the file questbook.htm ??
  • Do all of your links work?
  • Did you check your email to see if it worked?
  • Are all of your input areas working?
  • Is the CSS style sheet working?
  • Do you like the layout of your site? How might you change it?
  • Don't forget to send the instructor an email when you are done.
