III.  CATALOG DESCRIPTION

.This course is designed to teach personal and community health problems including mental health, drugs, alcohol, tobacco, nutrition, consumer education, personal safety and sex education.

A.  Course Advisory:  Tenth grade reading level on a standardized test, eligibility for English 1A or concurrent enrollment in English 52.

  IV.  REQUIRED TEXTBOOKS/MATERIALS

A.  Focus on Health by Payne and Hahn. (Ninth Edition or Ninth Edition). The text can be purchased at either the Weed or Yreka campus bookstores.(One copy of the textbook is on reserve at the COS Library.)

B.  Supplementary Materials

The COS Media Center has a variety of health videos that can be used for research purposes.

  VCOURSE OBJECTIVES 

A.  Students will be able to:

1.Comprehend, evaluate and analyze basic health issues concerning sexuality, drug abuse, mental health, nutrition, fitness, tobacco, personal health and alcohol.

2.Construct, analyze and defend individual strategies for successful health practices.

  VI.  COURSE PROCEDURE

A.  Chapter Assignments:

1.Read the assigned chapters and complete assignments or questions that are due.  (You must do the chapters, assignments, quizzes, papers, and tests in the order presented.)

2.It is beneficial to use the Review Questions at the end of each chapter to guide your reading in these chapters.  Many of the exam questions will be taken directly from these Review Questions.

B.  Quizzes:

1.Quizzes will be on Chapters 3, 7, 11, 13, and 16.

2.Quizzes are open book and must be taken in the order presented.  They are worth 20 points each.

C.  Exams:

1.Exam 1. covers Chapters 1 - 6.

2.Exam 2. covers Chapters 7 - 12.

3.Exams 1,2, and the final exam are open book. Students taking Health 10 online will take all exams via the online assignment page. Please refer to Exam Policy in the course syllabus.

D.Nutrition Assignment:

1.After reading Chapter 5 in the text you may complete the nutrition assignment.

2.Directions for the assignment are included with the assignment.

E.  Birth Control Assignment:

1.After reading Chapter 14 in the text you may complete the Birth Control assignment.

2.Directions are included with the assignment.

F.  Term Paper:

1.Read the directions that are included in the term paper requirements and complete the paper by the 5th week.

2.You are required to turn in a topic, introductory paragraph and two references and have them approved by the instructor prior to proceeding with the paper.

G.  Oral Interview Report:

1.The oral interview report it one of the most valuable assignments in this course.

2.Directions are included with the assignment.

B.  Testing Policy

 Student taking Health 10 online will take open book exams for tests 1and 2 via the Health 10 assignment page. REMINDER: DO NOT CLICK ON THE SUBMIT BUTTON AT THE END OF EXAM #1 OR EXAM #2. THE EXAMS MUST BE COPIED AND PASTED TO AN ORIGINAL EMAIL OR SENT AS AN ATTACHMENT. The final exam for online students will be taken online.

C.  Assignment Policy 

All assignments, quizzes and papers must be either copied and pasted to an original email or sent in as an attachment.

1)--Assignments must be done in the order they are presented. 

2)--Students can turn in only those assignments that are due during that week.

3)--Students can request to retake only one of the required exams.

4)--Students who fail an exam must meet with or contact by phone the instructor.

D.  Scheduling

Students wishing to complete this course should keep the following in mind:

1)--This course closely follows what is done in the semester long (17 week) Health in Action class. The online Health 10 course has been developed so that the course could be completed in a minimum of eight or a maximum of 16 weeks. Students taking the HEA 10 online  for Fall 2008 must complete the course in 8 weeks.

2)--Students must turn in a minimum of three assignments per week

3)--Sstudents will take the tests at the end of the 2nd, 4th, and 8th weeks.

4--Students should complete the term paper by the 5th week. 5)--Students should complete the Oral Interview assignment by the end of the 8th week. 1).

Assignment and Test Turn Around Time

I will have all assignments and exams back to the student in an expedient manner. (Most likely in 2-3 days.)

Total Points 860 GRADING SCALE:744 - 860 = A 688 - 743 = B 602 - 687 = C 473 - 601 = D 600 AND BELOW = F  VIII.  COS INTERNET GRADING POLICY After completion of an independent study course, the instructor will issue a final grade.The final grade will be posted to the student transcript at the end of the semester by the Admissions and Records Office.

A.  AVOID RECEIVING AN UNWANTED GRADE

Students have only 8 weeks to finish the internet course. Listed below are guidelines pertaining to students options.

1)ONE-SEMESTER REQUIREMENT .If a student finds they are unable to complete either course it is the student's responsibility to drop the course at the Admissions and Records Office before the "Last Date To Drop Without Penalty.  "If a student does not drop the course--a letter grade will be issued which could result in a "F" on the student transcript. 2)INCOMPLETE CONTRACT In a case of extreme emergency or family tragedy, a student may request an incomplete contract from the instructor.  Course work must be completed according to the timelines established in the incomplete contract.  If the timeline is not met the instructor will issue the grade earned at that time.

C. CHEATING POLICY

--Cheating will not be tolerated.  A student found cheating will be issued an F grade for the test or the assignment.  A second cheating offense will result in an F grade for the course.  

IX. TIME LIMIT All Health 10 courses must be completed in the semester they are taken. 

X. FINANCIAL AID/OTHER PROGRAM REQUIREMENTS Students receiving financial aid may only be paid once for an Outreach/Internet course.Contact the Financial Aid Office for specific regulations. NOTE: It is the student's responsibility to understand and abide by criteria set by Financial Aid or any other program in which they are participating.  

XI. STUDENT ASSISTANCE The instructor of record is available during their regularly scheduled office hours for questions regarding course content.  However, if students find they need help on a consistent basis, they need to utilize the Writing Lab, Math Lab, Business Lab, or tutorial services at the Weed campus.Also, students should check above in Section IV. A--Supplementary Materials for any suggested supplementary course materials.  A.  ACCOMMODATIONS Students needing special assistance or accommodations due to a disability may contact the Disabled Students Services Office in Eddy Hall I, (530) 938-5297.

If I feel that the topic is appropriate I may allow you to substitute the proposal for another agreed upon course requirement.
