Students requesting services through DSPS must provide documentation of their disability. This documentation must be signed by a licensed, or certified, professional. Documentation should describe the disability and explain any educational limitations.
In most cases, the documentation may not be older than three years and must come from an appropriate professional or agency.

When documentation of the student's disability has been received, an appointment with the Director/Counselor of DSPS will be made to discuss individual needs and accommodations. DSPS staff will maintain a current confidential e-file on each student receiving services.
