Spring Student Survey

Your voice matters! Share your feedback on Academic Success Center (ASC), Library, and Basecamp services and be entered into a drawing to win an iPad or a $50 gift certificate for either gas, Grocery Outlet, or Amazon (winner's choice). The survey will only take 5-10 minutes and is open from May 1 - 9, 2023

Your feedback helps to make our programs and services better by letting us know what is working well and where we can improve and provide more support. We are not always able to make changes right away but we are listening! Your feedback helps steer us in the right direction. Thank you.

Three is a second student survey happening right now. Participating will give you another chance to win a $50 Amazon gift certificate! Please note that this is a separate survey with a separate drawing and prizes.

Only one prize per person.

Grand Prize

The grand prize will be a new iPad (10.9-inch, 64GB, color: Silver).

Runner-Up Prizes

There will be two runner-up prizes: Each runner-up will receive a $50 gift certificate for gas, Grocery Outlet, or Amazon. The winner can choose which gift certificate they want.

How to Enter

All COS students currently enrolled on the day of the drawing (May 11) are eligible to participate in the survey and enter in the drawing. Permanent COS employees are not eligible (even if they are taking classes). The entry period is May 1 - 9, 2023.

The Student Survey has three sections: one for feedback on Basecamp, one for Library services, and one for the ASC. You must complete all three sections to be eligible for the drawing.

To enter, you supply your COS email address when prompted at the end of the survey (e.g. username@siskiyous.edu)

  • If you do not wish to enter the drawing, you can simply put "n/a" instead of your COS email address
  • You may only submit the survey and be entered into the drawing once
  • The survey is estimated to take 5-10 minutes
  • If you don't know your COS email address, please contact the Student Help Desk or the Library.

Entering/providing your email address is optional! You can still share your feedback without entering the drawing.

Anonymity

We encourage you to be honest when answering the survey. Your email address will not be used to match your name with your feedback. It is being used:

  • To select and identify the prize winners
  • As one contact method to contact the winners
  • To thank you for participating
  • To prevent fraud

The Drawing

The prize drawing will take place on May 11, 2023. Winners will be contacted via their COS email address; we may also contact you using the phone number and/or mailing address you have supplied to the college. A notice will be posted here once all winners have been contacted.

  • Only one prize per person
  • Prizes can be mailed to the winners if in-person pickup is not possible
